What no one is telling women over 40

No gatekeeping today.

There are conversations I have in clinic every single week that women in their 40s and 50s should have heard years ago.

Instead, you’re told: “It’s just aging.” “Your labs are normal.” “You're under a lot of stress.” Or maybe an antidepressant or GLP-1 is recommended. Meanwhile, you're struggling, and your body doesn't feel like your own.

So here it is. The things I wish every woman knew about perimenopause.


1) Iron infusions can be a game-changer

If your periods are heavy, long, or clotty, you are at real risk for iron deficiency.

Not just “a little tired.”

Low ferritin shows up as exhaustion, anxiety, hair shedding, shortness of breath, poor workouts, and restless sleep. And it often gets missed if hemoglobin is still normal.

If your flow is heavy, you need ferritin and hemoglobin checked regularly. It's not acceptable to struggle through 5-10 years with a ferritin below 20.

And yes - iron infusions can be appropriate and life-changing when indicated.

Stop normalizing depletion.


2) You can get a DEXA scan before 65

Bone loss and muscle loss start earlier than most women realize.

A DEXA scan doesn’t just assess bone density. It also shows visceral fat and lean muscle mass. That’s powerful information in your 40s and 50s that you can use proactively.

If muscle is declining or visceral fat is increasing, you want to know now - not after a fracture or a pre-diabetes diagnosis.

Midlife is about building your next decade, not reacting to the last one.


3) HRT can be an option in perimenopause

You do not have to wait until your period has been gone for 12 months. If you’re experiencing clear low-estrogen symptoms - hot flashes, night sweats, vaginal dryness, sleep disruption, mood instability - it’s reasonable to have a real conversation about hormone therapy.

Perimenopause is hormonal fluctuation. Some women benefit enormously from support during this phase.

And while we’re here - weight gain, fatigue, and brain fog are not always “just hormones.” Sometimes it’s blood sugar instability. Sometimes it’s muscle loss. Sometimes you’ve become more sedentary without realizing it. We have to look at the full metabolic picture, because estrogen won't solve all of your issues!


4) ADHD can flare - or finally get recognized

Estrogen supports dopamine. When estrogen fluctuates, focus and executive function can fall apart. Suddenly you feel scattered, overwhelmed, reactive.

Many women get treated for anxiety at this point, and if medication isn't helping much it's because it's not anxiety at the root - it’s unmasked ADHD.

If your brain feels different in this season, get assessed properly. The right diagnosis can change everything.


5) You can wear a CGM - but don’t obsess over a flat line

Continuous glucose monitors aren’t just for diabetes anymore. You can learn how your body responds to stress, sleep loss, strength training, wine, late dinners.

A couple of cautions however - the goal isn’t perfection, and it's not eliminating carbs. The goal is metabolic flexibility, and seeing where your blind-spots are. Data without context creates food fear. Guidance matters.


6) Bloodwork is still essential - even though we can’t “diagnose” perimenopause from it

Yes, hormones fluctuate wildly, so one test won’t define the transition. But that doesn’t make labs irrelevant. Relevant labs include: iron (ferritin), thyroid (TSH), lipid panel (cholesterol), HbA1c (blood sugar), Vitamin B12, Vitamin D, inflammation markers (C-reactive protein), and sometimes hormones.

Midlife symptoms overlap with many correctable issues. Don’t skip the data.


7) You have to exercise in this decade

This it the point in your life when exercise becomes a non-negotiable. Even if you've been able to get away without much movement in the past, your body will not tolerate that anymore. Exercise needs to target muscle (strength training), cardiorespiratory fitness (brisk walking at the very least), and mobility (stretching, yoga).

Without movement, metabolic health suffers and it can really catch up to you in perimenopause with weight gain, brain fog, mood swings, low energy and sleep disruption.

You don’t need extreme. You need consistent.


8) Vaginal dryness must be treated

This is not something to ignore or deal with it later. If you’re noticing dryness, irritation, discomfort with sex, or subtle burning - address it early.

Untreated dryness can progress to vaginal atrophy (genitourinary syndrome of menopause). The tissue becomes thinner, less elastic, more fragile, and it can be a lot of work to get it back.

And here’s what most women are never told: Recurrent UTIs in midlife are often caused by low estrogen in the vaginal tissue. When estrogen drops, the protective microbiome changes and the tissue loses resilience. If you’ve suddenly started getting UTIs “out of nowhere,” this is often hormonal.

Topical vaginal estrogen is safe for most women and works locally. It restores tissue health, it reduces UTI risk, it improves comfort, and it protects long-term function.

Please don't wait until sex becomes painful to ask for help. This is preventive care.


In Conclusion

You are not broken, and your body is not betraying you. Perimenopause and menopause are a transition.

When you understand what’s happening hormonally and metabolically, the chaos feels less personal - and far more manageable.
